Every specific cause you land on leads to the same checkpoint at the bottom of the map, where you can trace it one level deeper into either a Standards, Policies & Procedures gap or a Schedule & Production Pressure override — the two most common systemic reasons a cause actually happened.
All settings here are stored in this browser and apply to every investigation and NCR you work on.

Your project's investigations, NCRs, and settings live in Supabase, shared across everyone signed in to this project. Export a backup for an offline copy; restoring pushes the backup's contents back into Supabase.
